HHAx Reason and Action Codes

Overview

For states where HHAeXchange acts as an aggregator, action codes and reason codes are required on many visits.


Action codes are required to acknowledge actions taken. Reason codes identify the reason which caused the action. 


Both of these are available during the Approval process on the Visit Approval Dashboard. Visits which have EVV failures are flagged in Ankota and require this coding in order for the state to accept the visit.



Action Codes and Uses

Action codes are available per your state in Ankota. Common action codes include timesheet verified, updated member's address or phone number, and other.


Reason Codes and Uses

Reason codes are available per your state in Ankota. Common reason codes include caregiver failure to call in or out, mobile device issue, or timesheet received.
